Natriuretic peptide measurements as part of the diagnostic work-up in pleural effusions: an emerging concept?

چکیده
I n the 1950s and 60s, the possible role of the heart in the control of renal excretion of salt and water, and thereby of the extra cellular fluid volume, as well as the presence of specific granules within atrial muscle cells was recognised [1, 2]. Nevertheless, little attention was paid to the possibility of the existence of an endocrine cardiac natriuretic hormone system until the landmark experiments in the late 1970s by DE BOLD et al. [3], which led to the discovery of atrial natriuretic peptide (A-type natriuretic peptide; ANP) having potent natriuretic, diuretic and vasodilatator actions.

Cautious Application of Pleural N-Terminal Pro-B-Type Natriuretic Peptide in Diagnosis of Congestive Heart Failure Pleural Effusions among Critically Ill Patients
BACKGROUND AND OBJECTIVE Several studies on diagnostic accuracy of pleural N-terminal pro-B-type natriuretic peptide (NT-pro-BNP) for effusions from congestive heart failure (CHF) conclude that pleural NT-pro-BNP is a useful biomarker with high diagnostic accuracy for distinguishing CHF effusions. However, its applicability in critical care settings remains uncertain and requires further invest...
متن کاملHigh diagnostic accuracy of NT-proBNP for cardiac origin of pleural effusions.
A prospective study was performed to evaluate the diagnostic accuracy of N-terminal-pro-B-type natriuretic peptide (NT-proBNP) levels, measured simultaneously in serum and pleural fluid, in identifying pleural effusions due to heart failure. Pleural fluid and serum samples from all patients presenting for thoracentesis between April 2004 and May 2005 were simultaneously collected. The discrimin...

Background & Aim: Adenosine deminase(ADA) can be used in the diagnosis of tuberculous pleural effusions. In countries with high prevalence of tuberculous pleural effusions, specificity and sensitivity for ADA test is high, therefore it is an integral part of a diagnostic workup of lymphocyte-rich exudative body fluids, and it is a cheap and economically cost-effective test. No study has bee...
متن کاملTh1/Th2 Imbalance and Elevated PD-L1 in Pleural Effusion Predict the Risk of Multi-Drug Resistant Tuberculous Pleuritis
Background: Patient immune status might be indicative of the variance in bacterial genetics in drug-resistant tuberculous pleuritis and could be used for predicting the risk of multi-drug resistant tuberculous pleuritis (MDR-TB). Objective: To determine the significance of Th2/Th1 ratio and concentration of PD-L1 in the pleural effusions for prediction of MDR-T...
متن کاملPleural fluid amino-terminal brain natriuretic peptide in patients with pleural effusions.
BACKGROUND Definite diagnosis of transudative or exudative pleural fluids often presents a diagnostic dilemma. The aim of this study was to evaluate whether amino-terminal brain natriuretic peptide (NT-proBNP) levels in pleural fluid has a diagnostic value for discriminating heart-failure-related pleural effusions from non-heart-failure effusions. METHODS Sixty-six subjects (40 male, mean age...
